1. Data controller

The controller is Maria Owczarzak, MA, running a sole proprietorship under the name Maria Owczarzak Praktyka Psychologiczna, tax ID (NIP) 6762696919, REGON 542095311. Practice: ul. Dworska 8/10, Kraków, Poland. For anything about your personal data, write to mowczarzakpsycholog@gmail.com.

2. Contact form

What data: your name, e-mail address, phone number (if you give it), the message and your IP address, which is used to protect the form from spam.

Why: to reply to your message and arrange a session if you ask for one. Legal basis: Art. 6(1)(b) GDPR (steps taken at your request before entering into a contract) and Art. 6(1)(f) GDPR (replying to correspondence).

You don’t need to write about your health. A few words are enough, we will talk about the rest in person. If you do describe your situation, I will use it only to reply and keep it under professional confidentiality.

How long: I keep correspondence for as long as needed to stay in touch, no longer than 12 months after the last message. If we start working together, further data is kept under the rules on clinical records. The technical copy of each message kept on the website server, so that no message gets lost, is deleted automatically after 30 days.

Giving your data is voluntary, but without an e-mail address I cannot reply. I do not use profiling or automated decision-making.

3. Who else can access the data

  • Taro Media, the company that built and maintains the website and the contact form on my behalf. The server is located at Hetzner Online GmbH in Germany (EU).
  • Google Ireland Ltd., the Gmail e-mail provider that receives messages from the form. Google may process data outside the European Economic Area under standard contractual clauses.
  • ZnanyLekarz Sp. z o.o., ul. Kolejowa 5/7, 01-217 Warsaw, if you book through the calendar. Data you give when booking is processed by ZnanyLekarz as a separate controller, under its own privacy policy.

4. Cookies

The website itself does not use analytics or advertising cookies. It only uses your browser’s storage to remember your cookie choice and your place on the page when you switch language. Fonts are loaded from the website’s own server, with no connection to Google.

The calendar of available times comes from ZnanyLekarz and loads only after you agree. ZnanyLekarz may then set its own cookies and use analytics tools (for example Amplitude, Google Tag Manager, Sentry). Details are in the ZnanyLekarz cookie policy.

You can change your choice at any time: . Without consent you can still book directly on the ZnanyLekarz profile or write through the form.

Links to Instagram and Google Maps lead to external services with their own privacy rules.

5. Your rights

You have the right to access, rectify and erase your data, to restrict processing, to data portability and to object to processing based on Art. 6(1)(f) GDPR. You can withdraw cookie consent at any time. Just write to the address above.

You can also lodge a complaint with the Polish supervisory authority, the President of the Personal Data Protection Office (UODO), ul. Stawki 2, 00-193 Warsaw (uodo.gov.pl).
